What causes spine pain?

Neck and back pain often arise from soft tissue injuries like pulled muscles or ligaments. These injuries typically occur during athletic activities, while lifting a heavy object, or from a whiplash injury.

Overuse injuries are another common source of neck and back pain. When you repeatedly make the same movements, soft tissues can develop small tears. If you do not give your body time to heal, these tiny tears may progressively turn into more chronic injuries.

As you age, degenerative changes in the spinal joints and discs may lead to chronic neck and lower back pain. Common sources of spine pain include:

  • Facet joint arthritis
  • Thickened or weakened spinal ligaments
  • Herniated discs
  • Disc tears
  • Spinal stenosis
  • Degenerative disc disease
  • Sacroiliac joint sprains/strains/arthritis

All the conditions in this list can cause the sensation of a “pinched nerve”, a condition called radiculopathy. How does regenerative medicine address spine pain?

Platelet Rich Plasma (PRP) Therapy

Dr. Penhollow takes a sample of your blood and separates the platelets using a centrifuge to produce PRP. The PRP is precisely delivered under ultrasound and fluoroscopic (x-ray) guidance to the areas of injury and arthritis. After placing PRP at the injured tissues responsible for your neck or back pain, the platelets release growth factors. Growth factors then accelerate healing and regulate inflammation.

Adult Bone Marrow Derived Cell Therapy

Dr. Penhollow offers adult bone marrow derived stem cell therapy using autologous orthobiologics, which means she uses your own cells harvested from bone marrow in your upper pelvis.

This treatment, called bone marrow aspirate concentrate (BMAC) therapy, can produce results because adult bone marrow contains stem cells called mesenchymal stem cells (MSCs) as well as dozens of proteins containing growth factors and anti-inflammatory mediators. One of the most powerful is IRAP

Interleukin-1 (IL-1) is the inflammatory mediator that keeps arthritis going and keeps the joint in a degenerative state. Interleukin-1 receptor antagonist protein (IRAP) competes with IL-1 for the same receptor in the joint or disc, thus stopping the degeneration of the cartilage, capsule, and synovium (lining) and in the disc and soft tissue ligaments and muscles.

After Dr. Penhollow separates and concentrates the bone marrow aspirate in a centrifuge, she precisely delivers injects it under fluoroscopic (x-ray) guidance into the discs, epidural space, facet joints, sacroiliac joints and under ultrasound guidance into the spinal ligaments and muscles where the injuries and arthritis are.


During a prolotherapy treatment, Dr. Penhollow uses ultrasound to precisely deliver a solution containing a concentrated dextrose and a local anesthetic into the soft tissues including ligaments, muscles and fascia around the neck, middle back, and/or lower back. The dextrose solution activates the healing response, which in turn recruits circulating platelets and anti-inflammatory mediators in the bloodstream to the injured area.
